Welcome to our online registration form for the Collaboration to improve care (in our communities) programme. This is an innovative place-based leadership development programme that aims to address grounded, real-time challenges in the provision of joined-up care across health and care systems. This practical programme will be delivered over six months using a mix of virtual modules and small group learning sets focused on developing shared solutions to day-to-day issues. To register your place you just need to follow three simple steps after you have checked you can attend ALL dates outlined in your email:

Step Two: Please open and complete the baseline survey as part of the evaluation of the programme. By completing you are consenting for your name and email address to be shared with Innovas for the purposes of the programme evaluation only.
Step Three: Await joining instructions, which will be sent via email to you.
