DESCRIPTION:Techniques to embed Population Health Management approaches in day-to-day work \nThe NHS Long-Term Plan and People Plan has called for the integration care system (ICS approach including Primary Care and the evolution of Primary Care Networks (PCN). The ambition is to bring general practices\, dental\, optometry and community pharmacy together to provide coordinated healthcare solutions to the communities they serve. Learning from the challenges of operating within a Covid environment\, the need to develop the leadership capabilities of those within the PCN is a key priority to the delivery of the ICS agenda and tackling health inequalities. Population health management (PHM) is one of NHS England  core strategic aims for Integrated Care Systems; to improve physical and mental health outcomes\, promote wellbeing and reduce health inequalities across an entire population\, with a specific focus on the wider determinants of health (things like housing\, employment\, education). \nThe Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM\, with Primary\, Social and Secondary Care providers needing to risk stratify and proactively support key population groups with personalised care models. DESCRIPTION:This workshop is part of our Complete Leadership Series\, leading and managing teams\, to help you target areas of development leaders and managers have identified as top priorities right now. \nAbout the session  \nOur innate instinct to show compassion when we see others in distress or difficulty is an important part of what makes us human. At the same time\, it is also humanly possible to be selective with our compassion resulting in inequitable treatment and outcomes. This can be due to a range of different factors\, and during this workshop we explore\, what can impede compassion and also ways to expand our innate compassion to contribute to building inclusive mindsets\, practices and cultures. Learning how simple everyday changes can make a real difference.      \nThis workshop also provides those attending an opportunity to understand how microaggressions and microinvalidations can manifest and their impact. DESCRIPTION:Strategic Workforce Planning and Population Health Management Masterclass \nThe NHS Long-Term Plan and People Plan has called for the integration care system (ICS approach including Primary Care and the evolution of Primary Care Networks (PCN). The ambition is to bring general practices\, dental\, optometry and community pharmacy together to provide coordinated healthcare solutions to the communities they serve. Learning from the challenges of operating within a Covid environment\, the need to develop the leadership capabilities of those within the PCN is a key priority to the delivery of the ICS agenda and tackling health inequalities. Population health management (PHM) is one of NHS England core strategic aims for Integrated Care Systems; to improve physical and mental health outcomes\, promote wellbeing and reduce health inequalities across an entire population\, with a specific focus on the wider determinants of health (things like housing\, employment\, education). \nThe Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM\, with Primary\, Social and Secondary Care providers needing to risk stratify and proactively support key population groups with personalised care models.
