DESCRIPTION:Population Health Management and Health Inequalities \nThe NHS Long-Term Plan and People Plan has called for the integration care system (ICS approach including Primary Care and the evolution of Primary Care Networks (PCN). The ambition is to bring general practices\, dental\, optometry and community pharmacy together to provide coordinated healthcare solutions to the communities they serve. Learning from the challenges of operating within a Covid environment\, the need to develop the leadership capabilities of those within the PCN is a key priority to the delivery of the ICS agenda and tackling health inequalities. Population health management (PHM) is one of NHS England  core strategic aims for Integrated Care Systems; to improve physical and mental health outcomes\, promote wellbeing and reduce health inequalities across an entire population\, with a specific focus on the wider determinants of health (things like housing\, employment\, education). \nThe Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM\, with Primary\, Social and Secondary Care providers needing to risk stratify and proactively support key population groups with personalised care models.
