SUMMARY:Population Health Management and Health Inequalities Masterclass
DESCRIPTION:Population Health Management and Health Inequalities \nThe NHS Long-Term Plan and People Plan has called for the integration care system (ICS approach including Primary Care and the evolution of Primary Care Networks (PCN). The ambition is to bring general practices\, dental\, optometry and community pharmacy together to provide coordinated healthcare solutions to the communities they serve. Learning from the challenges of operating within a Covid environment\, the need to develop the leadership capabilities of those within the PCN is a key priority to the delivery of the ICS agenda and tackling health inequalities. Population health management (PHM) is one of NHS England  core strategic aims for Integrated Care Systems; to improve physical and mental health outcomes\, promote wellbeing and reduce health inequalities across an entire population\, with a specific focus on the wider determinants of health (things like housing\, employment\, education). \nThe Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM\, with Primary\, Social and Secondary Care providers needing to risk stratify and proactively support key population groups with personalised care models. PHM will become increasingly important as systems move to formally establish ICSs and will be pivotal to the way that the new systems will work together to improve the health of their populations. \nObjective of the session \nUnderstand the meaning\, and content of population health approaches in the context of system or ICS\nIdentify local\, regional\, and national opportunities for improving population health and evidence-based approaches (best practice).\nUnderstand the role of system to challenge their approach toward population health \nWho can attend the programme? \nEast of England region; South East region; London region; South West region; and Midlands region secondary (all system workforce)\, Social and Primary Care Networks including GPs\, ICS’s\, Pharmacy\, Optometry and Dental\, Paramedics\, ARRS roles\, community\, and practice nurses; midwives and health visitors; social workers\, family planning or sexual health clinics; out of hours GP services \nDate and Time: \nThis session will run on Zoom\, please scroll to the bottom to book your place on Tuesday 7 March  masterclass. Please note the session will also be repeated on the below dates. \nThursday 23 March 10.00-11.30\nTuesday 16 May 1.30-3.00\nWednesday 24 May 1.30-3.00\nThursday 1 June 10.00-11.30\nThursday 22 June 10.00-11.30\nWednesday 19 July 10.00-11.30 \nIs there a cost to participants? \nNo\, the programme is fully funded by NHS England

SUMMARY:The Patient Voice: Patient and Public Involvement
DESCRIPTION:As local NHS services transition to Integrated Care Systems and professionals adapt to working across regional boundaries\, organisations have been offered an opportunity to look at new and different ways to work to improve the overall health and wellbeing of people in their local area. This means it is more important than ever to enable patients and the public to get involved in service development. Patient and public involvement (PPI) is not only beneficial to services\, but also empowers individuals\, and leads to better relationships with the local community.  The aim of this masterclass is to provide delegates with a strong foundation on which to build their confidence in engaging patients and public within their professional practise. \nThis virtual masterclass will: \n\nExplore delegates’ current understanding and application of PPI\nProvide a regulatory background for PPI in primary care\nUse theoretical frameworks to explore how PPI benefits your service and individual practice\nShare the experiences of people who are empowered by PPI\nExplore established tools and techniques to improve PPI within your service\nDiscuss the challenges of PPI\nEnable delegates to identify and design meaningful involvement\n\nThe masterclass will be interactive and alongside learning new tools and techniques\, it will allow delegates to identify their own skills and knowledge base and share best practice with other professionals. It provides a supportive environment for professionals to ask questions about PPI and share any challenges they have experienced through previous engagement efforts. \nFacilitator: \nJen Rogers and Lauren Chaffey (Involve North East).  Involve North East\, are an organisation with over 20 years’ experience of PPI within Newcastle\, Gateshead\, and the wider North East region. They have an excellent reputation for delivering high quality engagement work to aid development of local NHS services. \nDate and Time: \n9 March 2022; 09.30-12.00 \nSpecial requirements: \nIf you have any special requirements\, please do not hesitate to get in touch with our team at midlands@leadershipacademy.nhs.uk

SUMMARY:Leading a Multidisciplinary Team Programme
DESCRIPTION:Strong leadership to develop cohesive and sustainable multi-disciplinary teams (MDTs) in a primary care setting is key to supporting and empowering service users within their own communities-being seen by the right person at the right time in the right place and supporting people to live their best lives. \nMDTs are the mechanism for organising and coordinating health\, social care and voluntary services to meet the needs of individuals with complex care needs. \nThis programme will support participants to develop their system leadership skills\, alongside developing an awareness and understanding of how to apply these skills to deliver true personalised care. \nThe offer will provide an opportunity for peer-led learning\, through facilitated Application Learning Groups. These groups\, in conjunction with the theory will support greater understanding of both the landscape they are working in\, as well as the challenges of developing a strong\, collaborative team of people from various professional backgrounds and team cultures\, who are coming together for the shared aim of putting the patient at the centre of the conversation. As the programme is pan-regional\, it will offer participants the opportunity of learning from colleagues across the country. \nThis is a pan-regional programme with a limited number of places. We are therefore keen to ensure that the right individuals have the opportunity of participating and that they have the support and sponsorship of both their line-manager and the wider system that they are working within. \nWho is this programme for? \nIndividuals responsible for developing\, facilitating and leading MDT working. \nProgramme Aims: \n\nExplore and understand the value of MDT working in the context of patient centred care\, population health\, placed based care and the wider landscape of Integrated Care Systems (ICS’s)\nUnderstand the importance of a shared vision and finding a common language. How to develop a shared vision and purpose across a multi-disciplinary team through co-creation and co-production\nTo identify and understand the enablers and barriers to effective MDT working\nTo develop wider system leadership skills in leading change\, developing trust\, building collaborative relationships\, changing mindsets\nTo explore the practical aspects of running a successful MDT-challenges and opportunities\nTo work with conflict – how to manage and resolve it and make it a positive part of team dynamics\nTo identify how to convey the impact and value of MDT working to the wider system-outputs\, impact\, measures of success\, storytelling\nSupport colleagues to spread learning across their PCNs and wider systems through a train the trainer approach\n\nThe programme is led and facilitated by experienced tutors from Tricordant who have significant experience of supporting and developing the Primary Care workforce. \nThroughout the programme the individual will have the opportunity to listen to and share ideas and experiences with their peers\, the facilitators and with guest speakers and subject matter experts in healthcare. \nProgramme Details: \nIn order to gain the most from the programme\, participants are expected to attend all sessions. There will be some pre-reading material to support theoretical knowledge and there will be an expectation that participants will reflect upon their learning and its impact on their practice with a view to exploring these reflections within their Application Learning groups. \n\n\n\nWorkshop/Application Learning Group\nWorkshop overview\nDates\n\n\nWorkshop 1\nThe WHY.
