SUMMARY:Impact of Long-Term Health Conditions
DESCRIPTION:This masterclass will consider the impact of long-term conditions from a public health perspective. Designed to support Primary Care Networks (PCN’s) and other health and care professionals working in population health\, participants will build their knowledge and skills within this area as well as: \n\nDevelop skills in strategic planning for community based programmes for chronic long-term conditions and its implications on public health\nInform health care workers\, carers and family members who live or work with people with chronic condition in self-managements and self-care to bring about life-long change.\nTo help people with long term chronic conditions regain their confidence\, mobility\, and return to exercise levels to improve and maintain their quality of life and functional independence\nTo get guidance on how to re-navigate their way through the changes in the Health and Social Care system that have occurred post COVID\n\nTopics to be covered within the session include perspectives on health (WHO\, Public health and Allied Health); social determinants of health in relation to chronic health conditions and health promotion and self-care strategies in long term health conditions. \nLearning outcomes:\n\nUnderstand population health in relation to long term chronic condition.\nBegin to understand the role that social determinates of health have on community based services in long term conditions.\nDevelop confidence in apply self-help & self management techniques in chronic long term conditions at primary care level.\nAbility to train others in self-help techniques in chronic conditions at primary care level.\n\nDate and times:\nMonday 3 October 2022; 10am – 12.00pm \nWho can attend the programme?\nAll NHS staff from Primary\, Secondary and Social Care \nIs there a cost to participants?\nNo\, the programme is fully funded by NHS England \nFacilitated by:\nDr Patricia Smith \nClosing Date:\nThis offer will close on the 3rd October (or earlier if places are filled). DESCRIPTION:Join us as Wellbeing expert\, Amber Sorrell\, gives tangible help and advice on how to approach conversations about mental health in the workplace. \nAmber covers what mental health actually is\, why it is important for businesses to consider and the difference between burn out and rust out.  She advocates for a person centric approach and has a unique toolkit to aid these conversations\, developed from her time as a detective in law enforcement. \nWho should attend? \nAnyone within the NHS.  Equality\, diversity and inclusion is about people and is important to all of us.  By raising awareness\, it promotes greater understanding and empathy and defines us and our values. \nPlease Note:- \nIf you need any special requirements with regards to accessibility\, either during\, or after the event\, then please don’t hesitate to contact us and we will do our best to assist. \nWe will send an email with details of joining instructions for the day to all participants\, prior to the date. \nIn order to participate you may need a webcam and a headset. \nYour computer will need to have the sound function on so you can hear and speak at the webinar. \nYou will need to be connected to the internet for the duration of the webinar. \nThis webinar may be recorded. \n  \nPlease also note that there will be a short evaluation form for you to complete at the end of the session – it is so important to us to get your feedback on each intervention we produce.  We want to ensure we continue to meet the needs and demands of the NHS.
