DESCRIPTION:“Communities of Practice are groups of people who share a concern or a passion for something they do and learn how to do it better as they interact regularly.” Etienne Wenger \nOur aspiration for the Midlands Community of Practice is to create a true community of system and organisational talent practitioners who have strong relationships\, shared ideas and collaborative working. This is a community for you and with you involved. Within the Community we aim to create a blueprint for great talent practice within the Midlands region and we need your voice\, input and collaboration to make this a true inclusive process for all of our Midlands organisations. \n  \nWhat is it about? \nThe session will be facilitated by Andrea Glover from Silvermaple\, and we also look forward to welcoming Rosie Smithson\, Head of Organisational Development at Norfolk Community Health and Care NHS Trust who has kindly agreed to share her experiences of embedding talent management practices.  The Trust’s approaches to talent mapping and succession planning were cited as a contributing factor in their ‘Outstanding’ CQC well-led rating. We will also be taking a look at succession planning including a quick guide to implementing different approaches as well as an opportunity to network\, share with and learn from organisation and system colleagues. \n  \nHow is it run? \nThis is a 150 minute virtual workshop . SUMMARY:Population Health Management and Health Inequalities Masterclass
DESCRIPTION:Population Health Management and Health Inequalities \nThe NHS Long-Term Plan and People Plan has called for the integration care system (ICS approach including Primary Care and the evolution of Primary Care Networks (PCN). The ambition is to bring general practices\, dental\, optometry and community pharmacy together to provide coordinated healthcare solutions to the communities they serve. Learning from the challenges of operating within a Covid environment\, the need to develop the leadership capabilities of those within the PCN is a key priority to the delivery of the ICS agenda and tackling health inequalities. Population health management (PHM) is one of NHS England  core strategic aims for Integrated Care Systems; to improve physical and mental health outcomes\, promote wellbeing and reduce health inequalities across an entire population\, with a specific focus on the wider determinants of health (things like housing\, employment\, education). \nThe Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM\, with Primary\, Social and Secondary Care providers needing to risk stratify and proactively support key population groups with personalised care models. SUMMARY:The Patient Voice: Patient and Public Involvement
DESCRIPTION:As local NHS services transition to Integrated Care Systems and professionals adapt to working across regional boundaries\, organisations have been offered an opportunity to look at new and different ways to work to improve the overall health and wellbeing of people in their local area. This means it is more important than ever to enable patients and the public to get involved in service development. Patient and public involvement (PPI) is not only beneficial to services\, but also empowers individuals\, and leads to better relationships with the local community.  The aim of this masterclass is to provide delegates with a strong foundation on which to build their confidence in engaging patients and public within their professional practise. \nThis virtual masterclass will: \n\nExplore delegates’ current understanding and application of PPI\nProvide a regulatory background for PPI in primary care\nUse theoretical frameworks to explore how PPI benefits your service and individual practice\nShare the experiences of people who are empowered by PPI\nExplore established tools and techniques to improve PPI within your service\nDiscuss the challenges of PPI\nEnable delegates to identify and design meaningful involvement\n\nThe masterclass will be interactive and alongside learning new tools and techniques\, it will allow delegates to identify their own skills and knowledge base and share best practice with other professionals. SUMMARY:Leading a Multidisciplinary Team Programme
DESCRIPTION:Strong leadership to develop cohesive and sustainable multi-disciplinary teams (MDTs) in a primary care setting is key to supporting and empowering service users within their own communities-being seen by the right person at the right time in the right place and supporting people to live their best lives. \nMDTs are the mechanism for organising and coordinating health\, social care and voluntary services to meet the needs of individuals with complex care needs. \nThis programme will support participants to develop their system leadership skills\, alongside developing an awareness and understanding of how to apply these skills to deliver true personalised care. \nThe offer will provide an opportunity for peer-led learning\, through facilitated Application Learning Groups. These groups\, in conjunction with the theory will support greater understanding of both the landscape they are working in\, as well as the challenges of developing a strong\, collaborative team of people from various professional backgrounds and team cultures\, who are coming together for the shared aim of putting the patient at the centre of the conversation. As the programme is pan-regional\, it will offer participants the opportunity of learning from colleagues across the country. \nThis is a pan-regional programme with a limited number of places. We are therefore keen to ensure that the right individuals have the opportunity of participating and that they have the support and sponsorship of both their line-manager and the wider system that they are working within. \nWho is this programme for? \nIndividuals responsible for developing\, facilitating and leading MDT working. \nProgramme Aims: \n\nExplore and understand the value of MDT working in the context of patient centred care\, population health\, placed based care and the wider landscape of Integrated Care Systems (ICS’s)\nUnderstand the importance of a shared vision and finding a common language. Learning from the challenges of operating within a Covid environment\, the need to develop the leadership capabilities of those within the PCN is a key priority to the delivery of the ICS agenda and tackling health inequalities. Population health management (PHM) is one of NHS England  core strategic aims for Integrated Care Systems; to improve physical and mental health outcomes\, promote wellbeing and reduce health inequalities across an entire population\, with a specific focus on the wider determinants of health (things like housing\, employment\, education). \nThe Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM\, with Primary\, Social and Secondary Care providers needing to risk stratify and proactively support key population groups with personalised care models. He has worked in and around health care for 35 years\, beginning as an Operating Department Orderly in 1981. In 2012\, he completed his Doctorate in Education at the University of Greenwich\, with a thesis that explored reflective practice and healthcare professionals through the lens of power. His first book appeared under the title Radical Organisation Development (Routledge\, 2020); more recently\, he co-wrote with John Higgins a book called Leadership Unravelled: The faulty thinking behind modern management (Routledge\, 2022). Their new title on voice\, silence and power in organisational life is due to be published early in 2023.
