SUMMARY:Leading Transformation for Integrated Care - Leading alongside your local communities
DESCRIPTION:Leading Transformation for Integrated Care \nColleagues working in health\, social care and the voluntary\, community and social enterprise sector are welcome to join us for our new series of 90-minute\, virtual sessions.  We have different topics for you to choose from which have been co-designed with the seven Regional Hubs of the NHS Leadership Academy and will be hosted by our delivery partner\, Tricordant. \nSo what is this series about?  Based on insights and experience drawn from work with a number of Integrated Care Systems across England these sessions will explore what partnership working looks like across teams and organisations. \nWhat does this mean for leaders?  It is co-designed with input from local representatives to understand their current challenges and opportunities. SUMMARY:Population Health Management and Health Inequalities Masterclass
DESCRIPTION:Population Health Management and Health Inequalities \nThe NHS Long-Term Plan and People Plan has called for the integration care system (ICS approach including Primary Care and the evolution of Primary Care Networks (PCN). The ambition is to bring general practices\, dental\, optometry and community pharmacy together to provide coordinated healthcare solutions to the communities they serve. Learning from the challenges of operating within a Covid environment\, the need to develop the leadership capabilities of those within the PCN is a key priority to the delivery of the ICS agenda and tackling health inequalities. Population health management (PHM) is one of NHS England  core strategic aims for Integrated Care Systems; to improve physical and mental health outcomes\, promote wellbeing and reduce health inequalities across an entire population\, with a specific focus on the wider determinants of health (things like housing\, employment\, education). \nThe Coronavirus (COVID-19) response to date has highlighted the pivotal role of PHM\, with Primary\, Social and Secondary Care providers needing to risk stratify and proactively support key population groups with personalised care models. PHM will become increasingly important as systems move to formally establish ICSs and will be pivotal to the way that the new systems will work together to improve the health of their populations. \nObjective of the session \nUnderstand the meaning\, and content of population health approaches in the context of system or ICS\nIdentify local\, regional\, and national opportunities for improving population health and evidence-based approaches (best practice).\nUnderstand the role of system to challenge their approach toward population health \nWho can attend the programme? \nEast of England region; South East region; London region; South West region; and Midlands region secondary (all system workforce)\, Social and Primary Care Networks including GPs\, ICS’s\, Pharmacy\, Optometry and Dental\, Paramedics\, ARRS roles\, community\, and practice nurses; midwives and health visitors; social workers\, family planning or sexual health clinics; out of hours GP services \nDate and Time: \nThis session will run on Zoom\, please scroll to the bottom to book your place on Thursday 23 March masterclass.
